To request a referral:
- use our online form
- phone us on 01924 223909 between 10am and 6pm
If you want a private referral letter:
- discuss the problem with a doctor, nurse or other healthcare professional
- book your appointment with your private provider
- use our online form or phone us on 01924 223909 between 10am and 6pm to provide us with the appointment details
Track a referral
You can track your referrals online by using the NHS referral tracker. You will need:
- booking reference number
- access code
You will be able to get these from the referral letter that was either e-mailed or posted to you.
Expediting referrals
Tell us if there is a change in your condition and we may be able to send an expedite letter. We cannot guarantee this will result in you being seen sooner.
Non-urgent advice: Chasing appointments
We are unable to chase appointments for referrals. You will need to contact the service on the number provided on the letter you were sent.
