To request a referral:

  • use our online form
  • phone us on 01924 223909 between 10am and 6pm

If you want a private referral letter:

  1. discuss the problem with a doctor, nurse or other healthcare professional
  2. book your appointment with your private provider
  3. use our online form or phone us on 01924 223909 between 10am and 6pm to provide us with the appointment details

Track a referral

You can track your referrals online by using the NHS referral tracker. You will need:

  • booking reference number
  • access code

You will be able to get these from the referral letter that was either e-mailed or posted to you.

Expediting referrals

Tell us if there is a change in your condition and we may be able to send an expedite letter. We cannot guarantee this will result in you being seen sooner.

Non-urgent advice: Chasing appointments

We are unable to chase appointments for referrals. You will need to contact the service on the number provided on the letter you were sent.
