You and Your General Practice charter
As part of the 2025/26 GP contract changes and the 10 Year Plan, NHS England has introduced You and Your General Practice (YYGP), also referred to as the Patient Charter.
From 1st October 2025, all GP practices are required to provide patients with access to this information.

The YYGP sets out what you, as a patient, can expect from your GP practice, alongside how you can help us to provide the best care for you and our wider community.
In line with the guidance, our practice has also nominated a YYGP Lead, our Admin and Reception Manager, who will act as a point of contact should you wish to provide feedback or raise any concerns about the charter.

Accessibility
We want to get better at communicating with our patients.
We want to make sure you can read and understand the information we send you. If you find it hard to read our letters or if you need someone to support you at appointments, please let us know.
- We want to know if you need information in large print or easy read.
- We want to know if you need a British Sign Language interpreter or advocate.
- We want to know if we can support you to lip read or use a hearing aid or communication tool.

Chaperones
This surgery is committed to providing a safe, comfortable environment where patients and staff can be confident that best practice is being followed at all times and the safety of everyone is of utmost importance.
All patients are entitled to have a chaperone present for any consultation, examination or procedure where they feel one is required. This can only be a trained member of staff which we will provide. It cannot be a friend or family member.
Wherever possible we would ask you to make this request at the time of booking your appointment so that arrangements can be made and your appointment is not delayed in any way. However if you would like a chaperone present at any time during a consultation when you are being examined please let the doctor, nurse or healthcare professional know, and we will do our best to organise for another member of staff to be present.
Confidentiality
We respect the need for confidentiality.
If you need a quiet place to speak away from the reception area, please let us know.
Medical records
The surgery complies with data protection and access to medical records legislation. Identifiable information about you will be shared with others in the following circumstances:
- To provide further medical treatment for you e.g., from district nurses and hospital services.
- To help you get other services e.g., from the social work department. This requires your consent.
- When we have a duty to others e.g., in child protection cases anonymised patient information will also be used at local and national level to help the Health Board and Government plan services e.g., for diabetic care.
Reception and Administration staff require access to your medical records to do their jobs. These members of staff are bound by the same rules of confidentiality as the medical staff.
To protect your confidentiality, we will not disclose any medical information over the telephone or email unless we are sure that we are talking to you. This means that we will not disclose information to your family, friends, and colleagues about any medical matters at all, unless we know that we have your consent to do so.

How we protect your data
Named GP
We assign all new and existing patients with a named accountable GP to oversee their care.
For patients aged 75 and over, the named accountable GP is responsible for:
- working with health and social care professionals to deliver a care package that meets the needs of the patient
- ensuring that these patients have access to a health check
Patients can still choose to see any GP in the surgery. We will make reasonable efforts to accommodate their preference.
